Billing

We accept some insurances.

You may require a referral for these appointments. Please have referrals prepared to: Laura Behm, NPI 1275-90-3916 / Tax ID 852088244. You may have a co-pay, coinsurance, or deductible. 

If we don't accept your insurance, payment will be required at the time of service. Medical appointments cost $150. We can provide a superbill for you to submit to your insurance.​
