Insurance Billing for Lactation:
Often, a referral for lactation services is not required, but this can depend on your specific insurance policy, particularly HMO policies. If a referral is needed, please have your referring provider locate our business using Tax ID: 85-2088244, and Providers: Laura Behm NPI 1275-90-3916 or Lea Lacefield NPI 1902-32-0971.
Many insurances cover our lactation services 100%, but billing through insurance does not guarantee reimbursement.
Since both the parent and child are patients, we submit two separate claims to insurance. We require insurance information for both the parent and the infant. Please notify us of any changes or updates to your insurance before your appointment.
Depending on your specific insurance plan, you may have a co-pay, deductible, or cost-sharing. If required by your insurance, a co-pay may be needed for both patients and is due at the time of the visit. We are legally obligated to charge these payments according to your insurance contract.
You may receive an Explanation of Benefits (EOB) from your insurance company. This is not a bill. If you have any questions, please contact our office directly as our team is happy to help. You can also review outstanding balance information in your patient portal.
Self-Pay Rates for Unpartnered Insurances:
We offer significantly discounted lactation rates for patients who are uninsured or underinsured.
If you have a private insurance carrier that we do not accept, we can provide a superbill for you to submit for reimbursement. Please note, we cannot guarantee reimbursement.
Mental Health Appointments:
Please bring a referral to your appointment. If you do not have a referral and it is required by your insurance, you will be charged the self-pay rate for the visits. Please have your referring provider locate our business using Tax ID: 85-2088244, and Provider: Laura Behm NPI 1275-90-3916.
The self-pay rate is $325 for an initial appointment and $150 for follow-up appointments. Payment is due at the time of service.
